The dental program uses a network of providers.  To receive the highest benefit level, you must use a network provider.  A list of providers in your area can be found by visiting the website at www.deltadental.com and following the directions in the link titled “Instructions to Find A Dentist” or by calling the toll-free number 800-524-0149.

The dental program is a voluntary plan.  This means that you can enroll in the dental plan even if you do not enroll in the medical program.  The rates and benefits are included in the links on the left.
